We think small dogs are the best pets in the entire world, however, they are not for everyone. We therefore request you complete the below application and answer some questions to help us determine what puppy will best fit your particular needs. Our puppies are bred solely for companionship, therefore they need an environment that will nurture them with lavish praise and attention. They do well with children, however children need to realize that little puppies need to be treated carefully as they do break! Bichons and Shih-Tzu's require frequent grooming and you must consider this in your monthly budget. Small dogs are sometimes quite stubborn, and can be difficult to housebreak, especially if you work all day, so you need to expect it may take as long as a year to completely housebreak your pet.

 If you are not certain you want a puppy from us, do not send a deposit!  Please do not ask us to hold a puppy for you if you do not intend to follow through with his adoption, or are continuing to shop other breeders.    Once you are confirmed on our wait list and we have cashed your deposit, we will not sell a puppy out from under you and you are confirmed in a specific order for the purchase of a puppy.  ie: 1st pick of liter, 2nd pick of liter, etc.   Unless otherwise stated, we will not consider you on our active  "wait list" without a deposit.  Note:  We reserve the right to keep a pick of the litter at any time and the right to reclassify a puppy to a "Tiny Tot".

 If you are not certain you want a puppy from us, do not submit a deposit! 

I ___________________________ am making a deposit of $300 this date for the purchase of a puppy from Fuzzypups!   Total adoption fee of puppy is $__________* plus tax plus, microchip transfer.   The deposit will apply to this total.  I understand that this fee is for the puppy only and does not cover charges for an out-of-state adoption, including shipping fees and health certificates. I further understand puppy must be paid in full before puppy will be released. 

    Note* adoption fee subject to change if we do not receive a deposit along with application.

I understand that by submitting this deposit, Fuzzypups! will put on hold a __________________________________ (sex/breed) puppy.   I understand there may be others ahead of me waiting for a puppy,  and that I am pick of the liter based on the date the check and paperwork are received and/or postmark date.         I understand that should I designate a specific gender and/or color, it could take more than one litter to fill my request.

I understand that if I choose not to take a puppy within a year of this deposit, the deposit will be forfeit, as it is not reasonable to expect Fuzzypups! to continue to wait on me for more than a year.
